At Alliance Senior Care we take a patient-centered and family centered approach. Just like everyone is unique, so are their care needs. We complete a free in-home assessment to determine a client’s care needs. In addition the a client’s care needs we will give the client the opportunity to discuss their preferences, values, beliefs, customs and cultural beliefs that need to be observed and respected when our staff is in their home.

Our Patient-Centered approach means that we are looking at the entire picture, including the patient’s family, neighbors, friends and support staff in this discussion as appropriate and feasible. Our goal at Alliance Senior Care is not to replace the family members but to support them throughout the process of supporting their family members.

Many seniors feel that if a caregiver is hired that their family will abandon them and stop providing the care and support they may have previously provided.

Just like everyone is unique, so are their care needs. In order to ensure that we are meeting each clients need, Alliance Senior Care creates a customized care plan for each client. Care plan includes the services that each client needs and wants. The personalized assessment helps ensure that there are no gaps in their care.

We start the interview process by conducting an in-depth assessment of you or your loved one’s lifestyle, medical conditions and the kind of assistance needed that will help them to remain independent at home. We will meet with you and your family and jointly determine the scope of care.
