Chronic Disease Management
Alliance Senior Care understands what it takes to care for a loved one with a chronic disease or multiple health conditions. We have developed specialized programs that target each patient’s need. Alliance Senior Care has specialized programs to care for the following chronic conditions:

Alzheimer’s and Dementia Home Care
Our staff are either CDP (Certified Dementia Practitioners) or CMDCP (Certified Montessori Dementia Practitioners). We see our client’s capabilities, not their deficits. Our goal is to provide caring support, cognitive enrichment, and engagement to provide the highest quality of life to our clients. In order to acheive our goal, we offer the following:
- Create a customized care plan based on client’s diagnosis, needs, and preferences
- Take a holistic approach, seek out non-pharmaceutical ways to handle behaviors
- Work with the family and/or the facility to create a positive and supportive environment
- Provide activities and engagement that are tailored to each client’s interest and capabilities
- Provide caregivers that are trained in Alzheimer’s and Dementia care
Transitional Care
Transitional care is designed to ensure a safe return home for individuals who have been:
- Discharged from hospital
- Discharged from Rehabilitation Center
- Post Operation
Transitional Care Services include all of the traditional care and support of our Home Care plus:
- Discharge planning assistance and support
- Ensuring that your loved one has the necessary equipment
- Ensuring the client obtains the prescribed follow-up care
Veteran’s Care
Are you a veteran or a surviving spouse of a veteran? Do you want to stay in your home but need some assistance?
- The VA offers assistance to help you pay for Home Care. If you are a wartime veteran or surviving spouse, 65 years or older, you may be entitled to this benefit
Alliance Senior Care offers special Services to assist Veterans and their surviving spouses, we:
- Help determine eligibility for VA and other benefits that will help cover the cost of home care
- Assist with completing the paperwork and navigating the VA system
- Support you in getting approved by the VA
